18-20 year old:

First time applicants will need to have 2 provider certifications from 2 different medical practices. 


*Homebound patients can receive 2 provider certifications from the same practice. 


18–20-year-old:

Renewal applicants prior to 2022 - 

only 1 certification required.

15 min exams = standard card exams, probation letters, uniform certification for current card holders, new or renewal patients.




30 min exams = patient's who need more time with physician to qualify for any increase in medication that will require Uniform Certification Form to purchase more than standard amount.

How do I apply?

We make it easy to obtain your medical marijuana card in just a couple of days!  Within 2-4 business days you will have your medical card and can easily start shopping at your local dispensary. 

You will need to call us first to set up an appointment with one of your M.D's for your exam/evaluation.
